When operating on the brain itself, it does not hurt. This is because the brain does not have pain fibres to convey the message. You can however cause pain by stimulating certain areas of the brain.
The scalp does have pain fibres so normally when you are being operated on there is a local anaesthetic which is used when cutting the scalp. It is useful that the brain doesn’t feel pain when being operated on, as that allows us to be able to see the effect of what we are doing to the person in surgery, without them being clouded by the “pain killers”.
