It all depends on the place of impact. It can affect fertility. I have seen a patient who was hit in that area when playing rugby. He ended up having to have a testicle removed. Sperm is still produced by the remaining one although if there were any issues like low sperm count to start with (he might not have known) or further trauma was caused, he could have issues.
There is the option of surgical sperm retrieval if a man can’t produce and ejaculate sperm normally. If this fails, the option left is donor sperm.
